Best and Worst Exercises for PCOS

Not every workout format carries the same benefit for PCOS. This guide breaks down which formats have the strongest support and which deserve a more thoughtful approach.

This guide reflects general training principles and is not medical advice. Exercise decisions for PCOS should be made in coordination with a physician or endocrinologist.

PCOS is one of the conditions in this series where the research on exercise is relatively clear about what helps most, even though individual response still varies. Insulin resistance is a core mechanism for a large share of people with PCOS, and exercise formats that improve insulin sensitivity carry the strongest support.

Resistance Training Has the Strongest General Support

Strength training consistently shows up in research as one of the more effective exercise interventions for PCOS, largely through its effect on insulin sensitivity and body composition. This is not a minor benefit tacked onto a cardio-focused program. It is a primary reason resistance training anchors most PCOS-focused programs rather than playing a supporting role.

A well-built strength program, two to four sessions a week covering major muscle groups, gives most clients the strongest general benefit available through exercise for this condition, independent of whether weight loss is part of the goal.

Why the Combination Matters More Than Either Alone

Research comparing resistance training alone, cardio alone, and a combination of both consistently favors the combined approach for insulin sensitivity outcomes in PCOS. This does not mean either format alone is without value. It means a program that includes both, rather than choosing one and skipping the other, tends to produce a stronger overall effect on the mechanism that matters most for this condition.

Moderate Cardiovascular Exercise Adds Real Additional Benefit

Walking, cycling, swimming, and other moderate cardio formats also support insulin sensitivity and general cardiovascular health, which matters given that PCOS carries some elevated cardiovascular risk factors for a portion of those affected. Combining moderate cardio with resistance training tends to outperform either approach alone in the research on this population.

There is no strong reason to avoid moderate cardio for PCOS, and it works well alongside strength training as a complementary piece of a well-rounded program.

Building a Progression That Respects Recovery

A practical approach for most clients starts with a foundation of moderate strength training and moderate cardio, consistent, sustainable, and well recovered from, for several weeks before introducing higher intensity elements. This gives a trainer a clear baseline for how the client responds to a manageable training load before adding a variable, high intensity work, that carries more individual variability in how it affects PCOS symptoms specifically. Building the higher intensity component in gradually, one session at a time, with close attention to sleep, energy, and general well-being in the days following, produces a much clearer picture of whether that format is helping or working against a specific client's goals.

High Intensity Training Deserves a More Careful Look

This is the area where the picture is genuinely less settled. Some research and a good amount of clinical experience suggest that very high intensity training can elevate cortisol in a way that may work against hormone balance for some people with PCOS, though this specific relationship is less consistently established than the general insulin-sensitivity benefit of exercise overall. Other people with PCOS tolerate and benefit from high intensity work without apparent issue.

Given that uncertainty, the practical approach is neither blanket avoidance nor blanket inclusion. It is introducing high intensity formats gradually, watching for signs like increased fatigue, disrupted sleep, or a general sense of feeling worse rather than better over a period of weeks, and adjusting based on what an individual client's actual response shows rather than a general rule applied universally.

Why Consistency Beats Perfection in Format Selection

A client who trains consistently with a slightly imperfect exercise selection will generally outperform a client who trains sporadically while chasing the theoretically optimal format for every session. This matters for PCOS specifically because the insulin sensitivity benefits of exercise depend heavily on regularity, a single great workout does not move the needle the way weeks of consistent, reasonably well-chosen training does. When in doubt between two reasonable format options, the one a client will actually stick with consistently tends to be the better choice over one that is marginally more optimal on paper but harder to sustain.

Formats That Deserve More Scrutiny

Very high volume, very high intensity training with minimal recovery, the kind sometimes marketed aggressively for fast weight loss, deserves real scrutiny for this population, both because of the cortisol consideration above and because inadequate recovery tends to undermine the consistency that actually drives results for PCOS over time.

Programs built entirely around cardio with minimal strength training miss the intervention with the strongest individual support for this specific condition. This is a common pattern in generic weight loss content and one of the more correctable mistakes in a PCOS-focused approach.

Any format pursued specifically to force rapid weight loss regardless of how the body responds tends to backfire for PCOS specifically, given the role stress and inadequate recovery can play in working against hormone balance for some clients.

The Real Answer Is Individual

PCOS presents differently across people, and how a given body responds to intensity, volume, and recovery varies enough that tracking an individual client's actual response matters more than applying a fixed formula. Two clients with the same diagnosis can have very different insulin resistance profiles, different baseline fitness levels, and different responses to the same training stimulus, which is part of why generic PCOS exercise advice found online often underdelivers compared to a program built and adjusted around one specific person. A program built around resistance training as the foundation, moderate cardio as a complement, and thoughtful, monitored introduction of higher intensity work produces better and more sustainable results than either avoiding intensity altogether or pushing it aggressively without tracking the response.

If energy crashes make consistency difficult, building a workout routine around PCOS energy crashes covers how to plan around that pattern without losing momentum.

Figuring out the right balance of intensity and volume for your body takes real tracking over real weeks, not a generic list applied blind, and the payoff for getting it right is meaningful given how directly the right formula connects to the underlying mechanism driving PCOS symptoms. I am a 42 year old woman who wants to continue onmy health and wellness journey. However, consistency is something I really struggle with, I usually start strong and fall off after a couple weeks. I started working with Reuben this past November and he talked to me about my goals and suggested powerlifting to me, honestly I thought he was crazy at first, me a 42 year old woman powerlifting. After some research, and the need to find something I could stick with, I said yes! The program that Reuben made me, his passion for fitness, and his encouragement has helped me to stay program tailored to my body and he took the time to figure out what areas I really needed to gain strength in. I would highly recommend Reuben for personal training! He's professional, passionate, and knowledgeable in what he does.

I've been training with Reuben for almost a year.  My results have been great.  I was never comfortable in a gym environment, but Reuben makes working out something I look forward to.  He's knowledgeable, explains exercises in a way that's easy to understand, and listens to my concerns.  He devises a program tailored to my needs.  I had old injuries that he is helping me strengthen.  I'm seeing results I wouldn't have thought possible.

Reuben has been a truly amazing guide in my fitness journey and in helping me attain my goals.  At this point I am just about halfway to my goal weight and I feel more confident and overall better in my day to day.  So much of that is owed to Reuben and his consistent efforts and dedication to help me reach my goals.  Each session is enjoyable and I don't feel like I'm training with a coach; I feel like I'm training with a friend.
